Here 3 NT shows the hand with four-clubs (no 2 ♥ rebid) and 18-19 hcp. This is excellent news for south with a good distributional hand. A bid of 4 ♥ would show five hearts and be offering a choice between hearts and no trumps. However with four clubs opposite a slam in clubs is likely to be better. On some auctions there might be a possibility to offer hearts later. Possibly here with great hearts it would be better to show the fifth heart. Clubs is likely to be better only when there is a club ruff going in 6 ♣ (and 6 ♣ can be made).

Imagine if north had the ♦ A rather than the ♦ KQ. Now 7 ♣ would be good but it would be harder to get there after we waste a level by exploring for the heart fit.
